HIPAA-Compliant File Transfer Service

You can now send documents containing sensitive data to Your Amazing Journey via a HIPAA-compliant file sharing service. Your Amazing Journey now uses Filemail to transfer files. Any documents listed in the Forms section can be uploaded below.

INSTRUCTIONS

This is a HIPAA-compliant service. Your data is protected. PLEASE whitelist no-reply@filemail.com so that you will receive emails.

PLEASE DO NOT USE THIS SERVICE ON A MOBILE PHONE AS IT IS DIFFICULT OR NOT DOABLE. UPLOAD YOUR FILES FROM YOUR DESKTOP OR LAPTOP COMPUTER.

The following documents must be uploaded in PDF format: Application for Services

  • Employment & Education History form
  • Signed YAJ SSW Communication Policy form
  • Signed P2P SMS Consent form
  • SSA-3288 Consent for Release of Information
  • YAJ Release of Information

The following documents should be MS Word files in .docx format:

  • Resume’

The following documents must be in an image format: .jpg, .jpeg, or .png:

  • ADEN Profile Photo (color, head/shoulders)

Assessments

  • The file format will vary deping on the assessment (e.g., PHQ-9, O*NET Inventory, etc.) I will notify participants of the correct file format for each assessment if necessary.

Pay Stubs:

  • Pay stubs must be in PDF format. Please do not combine stubs in a file. One stub per file please.
  • Your pay stub must be uploaded each day you are paid: weekly, or every other week if you are paid bi-weekly.
  • Please write your SSN clearly on each stub in the blank space by your name. Do not write in the margins of the page.

End of Month Reporting:

TO SSA:

  • You, the participant, are responsible for faxing all pay stubs for each month to your local SSA office before the 6th of the following month. If you have a stub in the new month that falls before the 6th, include that stub with your fax.
  • Use the fax cover sheet provided to you.
  • Make sure your SSN is on all your stubs.
  • Check your stub for the types of hours to make sure all hours are clearly identifiable as earned or unearned income. Regular, overtime, bonuses, and commissions are earned income. Annual, vacation, sick, and PTO are unearned cinome. An example of hours unclearly marked is Other. In such a case, make sure to indicate on your fax cover sheet and the stub whether the hours are earned or unearned income and explain what it is for. E.g, Pay date 1/1/24 16 hours for $400 marked Other is unearned income for inclement weather.
  • Please wait for a fax receipt and keep all fax receipts for your records!

TO YOUR AMAZING JOURNEY, LLC:

  • After you complete your fax to SSA, please upload your fax cover sheet that you sent to SSA. I keep these so I have a record of when you fax to SSA, the pay dates of the stubs you fax, and any notes you make about your hours. I use this information for your monthly BWIA (Benefits & Work Incentives Advising).
  • If I’ve asked you to track your work hours, please also upload your work calendar. Include hours for any days you worked that fall outside the beginning and end of the month that show on the calendar. This helps me to provide a more accurate analysis. For example, for February 2024, you would write in any hours you worked in February and also any hours you worked on January 28, 29, 30 and 31, and March 1 and 2.

How to Upload Files:

  • Enter your email address in the “From (email)” field below.
  • Add a short message in the “Message” field describing what you are uploading. E.g. “Here is my completed application,” or “Pay stubs attached,” etc.
  • Drag and drop your file(s) from your computer into the Messsage field using your mouse. Or use the purple “Add Files” button to browse your computer for your file(s). Make sure your files are attached. The document names will show.
  • Click the “Send” button and wait for the pop-up message, “The files have been successfully sent!”
  • Click “Done”
